Support Worker
About the role:
We are looking for compassionate, and motivated individuals to join our Outreach Team in Wakefield. This is a fantastic opportunity to make a real difference in the lives of people who need it most.
As a Support Worker within the Outreach Team, you will be responsible for supporting individuals with Autism, learning disabilities or physical disabilities. You will work to empower the people we support to live independently, improve their well-being, and connect with their local community.
Key Responsibilities:
- Assist with practical tasks such as cleaning, budgeting, and managing appointments.
- Offer emotional support, build trust, and encourage independence in individuals.
- Develop and maintain positive relationships with clients, working closely with other professionals and agencies to coordinate care.
- Conduct assessments and support planning, ensuring the needs of each individual are met.
- Monitor and record progress, ensuring accurate and up-to-date documentation in line with policies and procedures.
- Promote community integration and wellbeing by assisting individuals in accessing community activities and social opportunities.
A driver with a full UK driving licence is required for this role.
Hours:
We have full & part time positions available. Working Monday to Friday between 9am-7pm.
Some weekends shifts may be required depending on the needs of the people we support.
Location:
Our outreach team work across Wakefield and the surrounding areas.
